ANNEX 2 - Conditions Consistent With The Operating Schedule
General
All staff shall be suitably trained for their job function in the operating standards for the premises and the contents of the premises licence.
The staff training will include the identification and recognition of drunks and the correct procedure to be followed when refusing service. Such training shall be recorded and refresher training given annually.
Prevention of Crime and Disorder
A tamper proof CCTV system shall be installed, recording and maintained in working order and operated at the premises. All areas the public have access to in the premises (including the toilets), shall be covered by the CCTV system and to the satisfaction of Lincolnshire Police, specifically:
a) There shall be a minimum of one high resolution camera fitted in a weatherproof housing, for external coverage of any entrance.
b) There shall be a minimum of one high‐resolution colour camera fitted to the public entrance/exit to provide a quality head and shoulder image for facial recognition/identification purposes of all persons entering the premises.
c) There shall be sufficient cameras able to cope with the normal operating illumination to reasonably cover all licensed public areas.
d) Recordings must be kept for a minimum of 28 days and endorsed with the accurate, correct time/date (BST/GMT adjusted).
e) The frame rate shall be maintained to a standard whereby any footage produced is of sufficient quality to be of evidential use.
f) Playback of any incident without the necessity for download must be possible.
g) A member of staff should be on the premises at all times they are open to the public who is capable of operating the CCTV system and providing recordings on request. When this is not possible recordings will be provided within 24 hours of the original request
h) Recording equipment shall be housed in a secure room/cabinet where access and operation is strictly limited to authorised persons.
There shall be provided at the premises door supervisors who are registered with the Security Industry Authority (SIA) to such a number as the management of the premises consider are sufficient to control the entry of persons to the premises and for the keeping of order in the premises when they are used for a licensable activity.
When regulated entertainment is provided after midnight a minimum of 2 door supervisors will be employed at the premises until close of business.
There shall be provided and kept at the premises a register/log book to record details of every person employed on the premises as a door supervisor. The record shall contain the following details:
· The door supervisor's name;
· His/her Security Industry Authority (SIA) licence number;
· The time and date he/she commenced and concluded their duty;
· The door supervisors shall sign each entry;
· The Designated Premises Supervisor or other authorised person shall also endorse each entry as having checked the authenticity of the individual door supervisor.
The log to be retained for a period of no less than 12 months and made available to Lincolnshire Police for inspection upon request.
An incident book shall be kept at the premises in which details of incidents of crime and/or disorder relating to the premises shall be recorded. The incident book shall contain the following details:
· Time, date and location of incident.
· Nature of the incident.
· Names, addresses and contact details of persons involved.
· Result of the incident.
· Action taken to prevent further such incidents.
Each entry signed by the DPS or other responsible person employed at the premises and so authorised by the DPS.
The incident book shall be made available to the Police upon request. Each entry shall be retained for a period of 12 months from date of completion.
A sales refusals book, or electronic record, shall be kept to record all instances where service of alcohol is refused. Records to be retained for at least 12 months, and shall be made available for inspection to the Police, or other officers of a Responsible Authority, upon request.
There shall be a drug policy completed for the premises. This document will be specific to the venue and will be kept on site at all times and will be made available for inspection to the Police, or other officers of a Responsible Authority, upon request.
Drug awareness training will be provided to all employees. The training shall include an agreed procedure for dealing with persons who are in the possession of drugs or incapacitated through the use of drugs (or incapacitated through the combined effect of drugs and alcohol). The training shall be under constant review.
Public Safety
The premises licence holder shall ensure that there is an adequate risk assessment in place in regards to the balcony which shall include measures to address the risk of glass being dropped from the balcony onto people below and the use of door supervisors.
The risk assessment shall be written down and kept at the premises and be available for production on demand by a person under the direction and control of the Chief Constable of Lincolnshire Police or an authorised Officer of East Lindsey District Council.
No sales of alcoholic beverages will take place until a Policy and/or risk assessment in relation to when Polycarbonate drinking vessels will be used is produced, presented and agreed by Lincolnshire Police.
Where available any glass drinking vessels shall be toughened glass.
No glass drinking vessels or glass bottles shall be permitted to leave the Grand Parade Leisure Complex.
The supply of alcohol for the consumption off the premises is limited to the consumption within areas of the Grand Parade Leisure Complex only.
There shall be no admission to the premises from 03.30 hours until close, save for customers who are already within one of the facilities within one of the facilities within the Grand Parade Leisure Complex or who may have temporarily left the premises (and can be identified by way of a stamp or other method). This is to mean that there shall be no admission to the Grand Parade Leisure Complex after 03.30 hours, except for the Food Court area, until close, save for customers who are already within one of the facilities within the Grand Parade Leisure Complex or who may have temporarily left the premises (and can be identified by way of a stamp or other method).

The Protection of Children from Harm
A Challenge 25 proof of age policy to be applied at the premises with the accepted means of proof being:
• Passport
• Photo Driving Licence
• A recognised valid photo-id card bearing the PASS hologram
• Any future accredited and accepted proof of age as defined by Lincolnshire Police.
All point of sale staff shall undergo training in the above policy with a record kept of the date of training, signed by the member of staff and the trainer. Each entry shall be retained for a period of 24 months from date of completion. This record shall be made available for inspection by Lincolnshire Police Officers or other parties acting on their behalf.
No persons under the age of 16 shall be permitted on the premises after 23.30 hours until close of business on any day of the week.
