ANNEX 2 - Conditions Consistent With The Operating Schedule
General
All staff shall be suitably trained for their job function in the operating standards for the premises and the contents of the premises licence.
The staff training will include the identification and recognition of drunks and the correct procedure to be followed when refusing service. Such training shall be recorded and refresher training given annually.
The Prevention of Crime and Disorder
A tamper proof CCTV system shall be installed, recording and maintained in working order and operated at the premises to the satisfaction of Lincolnshire Police, specifically:
a) There shall be a minimum of one high resolution camera fitted in a weatherproof housing, for external coverage of any entrance.
b) There shall be a minimum of one high resolution colour camera fitted to the public entrance/exit to provide a quality head and shoulder image for facial recognition/identification purposes of all persons entering the premises.
c) There shall be sufficient cameras able to cope with the normal operating illumination to reasonably cover all licensed public areas.
d) Recordings must be kept for a minimum of 28 days and endorsed with the accurate, correct time/date (BST/GMT adjusted).
e) The frame rate shall be maintained to a standard whereby any footage produced is of sufficient quality to be of evidential use.
f) Playback of any incident without the necessity for download must be possible.
g) A member of staff should be on the premises at all times they are open to the public who is capable of operating the CCTV system and providing recordings on request. When this is not possible recordings will be provided within 24 hours of the original request
h) Recording equipment shall be housed in a secure room/cabinet where access and operation is strictly limited to authorised persons.
All areas the public have access to in the premises (including the toilets) shall be covered by the CCTV system.
There shall be provided at the premises door supervisors who are registered with the Security Industry Authority (SIA) to such a number as the management of the premises consider are sufficient to control the entry of persons to the premises and for the keeping of order in the premises when they are used for a licensable activity.
When regulated entertainment is provided after midnight a minimum of 2 door supervisors will be employed at the premises until close of business.
There shall be provided and kept at the premises a register/log book to record details of every person employed on the premises as a door supervisor. The record shall contain the following details:
· The door supervisor's name;
· His/her Security Industry Authority (SIA) licence number;
· The time and date he/she commenced and concluded their duty;
· The door supervisors shall sign each entry;
· The log to be inspected weekly and signed by a member of the management team.
The log to be retained for a period of no less than 12 months and made available to Lincolnshire Police for inspection upon request.
An incident book shall be kept at the premises in which details of incidents of crime and/or disorder relating to the premises shall be recorded. The incident book shall contain the following details:
· Time, date and location of incident.
· Nature of the incident.
· Names, addresses and contact details of persons involved.
· Result of the incident.
· Action taken to prevent further such incidents.
Each entry to be signed by the Designated Premises Supervisor (DPS) or other responsible person employed at the premises and so authorised by the DPS. The incident book shall be made available to the Police upon request. Each entry shall be retained for a period of 12 months from date of completion.
The premises licence holder shall ensure that a sales refusal register, or electronic record, is maintained at the premises. The refusal register to include details of all alcohol sales refused and reason for refusal.
Records to be retained for at least 12 months, and shall be made available on request to a Police Officer or other Officers of a Responsible Authority, upon request.
There shall be a drug policy completed for the premises. This document will be specific to the venue and will be kept on site at all times and will be made available for inspection to the Police, or other Officers of a Responsible Authority, upon request.
Drug awareness training will be provided to all employees. The training will include an agreed procedure for dealing with persons who are in the possession of drugs or incapacitated through the use of drugs (or incapacitated through the combined effect of drugs and alcohol). The training shall be under constant review.
Public Safety
If there are 25 or more customers on the front balcony smoking area when regulated entertainment is taking place then between midnight and close of business, no glass drinking vessels shall be permitted on the front balcony smoking area.
No sales of alcoholic beverages will take place until a Policy and/or risk assessment in relation to when Polycarbonate drinking vessels will be used is produced, presented and agreed by Lincolnshire Police.
Where available any glass drinking vessels shall be toughened glass.
No glass drinking vessels or glass bottles shall be permitted to leave the Grand Parade Leisure Complex.
The supply of alcohol for the consumption off the premises is limited to the consumption within areas of the Grand Parade Leisure Complex only.
There shall be no admission to the premises from 03.30 hours until close, save for customers who are already within one of the facilities within one of the facilities within the Grand Parade Leisure Complex or who may have temporarily left the premises (and can be identified by way of a stamp or other method). This is to mean that there shall be no admission the the Grand Parade Leisure Complex after 03.30 hours, except for the Food Court area, until close, save for customers who are already within one of the facilities within the Grand Parade Leisure Complex or who may have temporarily left the premises (and can be identified by way of a stamp or other method).

The Protection of Children from Harm
A Challenge 25 proof of age policy to be applied at the premises with the accepted means of proof being:
• Passport
• Photo Driving Licence
• A recognised valid photo-id card bearing the PASS hologram
• Any future accredited and accepted proof of age as defined by Lincolnshire Police.
A notice or notices shall be displayed in and at the entrance to the premises where they can be clearly seen and read and shall indicate:
a) That it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person under 18 years of age.
b) That proof of age may be requested at the premises and those accepted means for proof of age listed as above.
No persons under the age of 16 shall be permitted on the premises after 23.30 hours until close of business on any day of the week.
